Do doctors need GST registration in India?

  • The threshold rule: If your annual turnover from taxable medical services exceeds ₹20 lakh (₹10 lakh in special category states), GST registration is mandatory.
  • What is taxable: Consultation fees, diagnostic services, dental procedures, cosmetic treatments, and non-essential surgeries are taxable at 18% GST.
  • What is exempt: Services by a clinical establishment to a patient for diagnosis or treatment are exempt under GST Notification 12/2017.
  • The grey area: If you are a senior consultant attached to a hospital, the hospital's billing may cover you. But if you run your own clinic, you likely need GST registration above the threshold.
  • When in doubt: Consult a GST practitioner. The cost of wrong classification is far higher than the cost of professional advice.

What every medical invoice should include

  • Doctor/Clinic name and credentials: Dr. [Name], [Qualifications], [Registration Number with Medical Council].
  • Clinic address and contact: Full address, phone number, and email. Insurance companies need this for verification.
  • Patient details: Full name, age, gender, and patient ID. For insurance claims, the patient's name must match their policy.
  • Date of service: The date when consultation or procedure was performed.
  • Service description with medical codes: Use standard terminology. 'Consultation — General Medicine' or 'Root Canal Treatment — Tooth #36'.
  • Breakdown of charges: Consultation fee, procedure fee, medication, consumables, room rent if applicable.
  • GST details (if registered): SAC code (usually 998311 for healthcare services), GST rate (18% for most non-exempt services), and GST amount.
  • Total amount in figures and words: Required for GST compliance.
  • Payment method and status: Cash, Card, UPI, or Insurance. Mark 'Paid' or 'Pending' clearly.
  • Doctor's signature: Wet ink or digital signature.

OPD vs IPD billing — different approaches

  • OPD (Outpatient) billing: Single-visit services. Issue the invoice immediately after consultation or procedure.
  • IPD (Inpatient) billing: Multi-day hospitalization. Create a running bill that is updated daily. Final invoice is issued at discharge.
  • Package deals: For surgeries or treatment packages, issue a proforma invoice first showing the package inclusions and exclusions.
  • Emergency services: Bill separately with clear marking 'Emergency Consultation/Procedure'.
  • Follow-up visits: If included in the initial consultation fee, state 'Follow-up within 7 days — No additional charge'.

Insurance and TPA billing — what doctors must know

  • Cashless vs Reimbursement: For cashless, the TPA pays you directly. For reimbursement, the patient pays you and claims from insurance.
  • Pre-authorization requirements: For planned procedures, get pre-auth from the insurance company before treatment.
  • Mandatory fields for insurance: Patient name (as per policy), policy number, TPA name, date of admission and discharge, diagnosis with ICD codes.
  • Discharge summary linkage: Your final invoice should reference the discharge summary number.
  • Common rejection reasons: Mismatched patient name, missing signatures, non-itemized bills, services not covered under policy.

  • UPI QR codes at reception: Patients can scan and pay immediately. Reduces cash handling.
  • Card machines (POS): Essential for higher-value procedures.
  • Payment links via SMS/WhatsApp: For teleconsultation or follow-up payments.
  • Online appointment platforms: If you use Practo, Lybrate, or similar, understand their billing cycle.
  • Reconciliation: Match daily collections against invoices issued.
